Authorized Person Authorized Person ………./……….…../……… ………./……….…../……… Conditions 1 This Permit will be displayed openly at a location of regulated energy production. 2 This permit is usable only at a location of regulated energy production as described herein. 3 A permitted person must obtain an approval by the Department of Energy Development and Promotion prior to removal, change or modification of equipment, being used in regulated energy production, from that appeared in an appendix to its application form. 4 A permitted person shall maintain equipment used in regulated energy production as well as their measuring devices in functional condition at all times. 5 Upon receipt of a form of annual report from the Department of Energy Development and Promotion, a permitted person shall fill in and send it, with relevant documents as described therein, to the Department of Energy Development and Promotion within the month of January of the following year. 6 In case of violation or no compliance with above conditions of Clause 1 to Clause 4, the Department of Energy Development and Promotion may give a warning, suspend or revoke a permit when considered suitable. Warning The permitted person shall submit an application for renewal of a permit of regulated energy production, not less than 120 days prior to its expiry date.
