The t·etttrn requil"tJJ by si.'etion 19 of the Act shall be in the form set out in Sehcdulc Si:-:: hcr()to and s'.1c1l1 contain all i11formation indicated as required in the said form. 10. (1) A person engaged in operations for the drilling or con­ stnrntion of a well for which a permit has been issued shall keep the following- records and sampleS and shall, within 14 days of the completion or abamlonment of such work, forward the records, copies of which Hhall hr .~npplied tn thr pPrmit holrlf'r, imd samplf'~ to t.hr :Minister, provided however, that the Minister or an ;inthorized pct';,'.Ott m,1,v at any time inspeet the reconls and samples or request that they be forwarded to the Minister ut such other time as he may think fit: (al Representative s.umples of not l~~s than ½ lb. weight taken at each change of strata observed in the well, and in any case at intervals not exceediug 10ft. of advance of the well. Such samples to be enclosed in a suitable container, and to be cl.early marked so as to identify the well and depth at which the sample was taken. (1) Application for a Well Driller's Licence shall be ma<lc in the form set ont in S.::hcdule Elght hereto, and shall contain all information indicated as required in the said form. (2) The prcscribe<l. !ee for a Well Driller's Lic<incc shall be $10 and shall be payable upon the makmg of the apphcation for the licence, but shall be refut1d£:d if the application is refused, (3) The p1·eSl'ribed fee fol* the renewal of a 1-Ve/1 Dri11er's Licence shall be $10.
