Regulation on application forms for consents and permissions for activities concerning genetically modified organisms

Dziennik Ustaw Nr 87 — 5766 — Poz. 797 797 ROZPORZÑDZENIE MINISTRA ÂRODOWISKA z dnia 6 czerwca 2002 r. w sprawie okreÊlenia wzorów wniosków dotyczàcych zgód i zezwoleƒ na dzia∏ania w zakresie organizmów genetycznie zmodyfikowanych. Na podstawie art. 22 ustawy z dnia 22 czerwca 2001 r. o organizmach genetycznie zmodyfikowanych (Dz. U. Nr 76, poz. 811 oraz z 2002 r. Nr 25, poz. 253 i Nr 41, poz. 365) zarzàdza si´, co nast´puje: § 1. Rozporzàdzenie okreÊla wzory wniosków: 1) o wydanie zgody na: a) zamkni´te u˝ycie organizmów genetycznie zmo dyfikowanych, zwanych dalej „GMO”, stano wiàcy za∏àcznik nr 1 do rozporzàdzenia, b) zamierzone uwolnienie GMO do Êrodowiska w celach innych ni˝ wprowadzenie do obrotu, stanowiàcy za∏àcznik nr 2 do rozporzàdzenia, 2) o wydanie zezwolenia na: a) wprowadzenie do obrotu produktów GMO, sta nowiàcy za∏àcznik nr 3 do rozporzàdzenia, b) wywóz za granic´ produktów GMO, stanowiàcy za∏àcznik nr 4 do rozporzàdzenia, c) tranzyt produktów GMO, stanowiàcy za∏àcznik nr 5 do rozporzàdzenia. § 2. Rozporzàdzenie wchodzi w ˝ycie po up∏ywie 14 dni od dnia og∏oszenia.
